Do you want to contribute to ensuring that children and young people with complex needs receive safe and coordinated help? As SIP coordinator, you get a central role in creating structure, clarity and cooperation between the actors around the child.

Welcome to us

We are one of Stockholm's 11 district administrations, with offices in the Slakthus area near Globen. In the assignment, you work both towards:

The client disability unit, where you meet children and young people with disabilities, NPF or need support according to LSS and SoL.
Units within individual and family care, children and young people, where the focus is on social and psychosocial problems, interventions according to SoL and LVU and support for families.

You are organizationally placed under the area manager for Children and Young People. The project employment lasts for two years with the possibility of extension.

Your role

You lead the work with coordinated individual plans (SIP) for children and young people who need interventions from several activities, for example:

Mental illness
Neuropsychiatric impairments
Comorbidity
Efforts from both social services and the region

The coordination also includes guardians and networks. You keep the process together and ensure that the right people are involved, including from:

Regional care (BUP, psychiatry, habilitation)
Preschool, school and student health
Social service units
Other support activities such as HVB, outpatient care and family centres.

You:

accepts assignments from social workers and aid workers
initiates, leads and follows up SIP meetings
ensures that the right actors participate
responsible for documentation and follow-up
provides advice on SIP issues
contributes to the development of routines and collaboration structures
trains internally and externally.

The assignment is followed up annually, among other things through the children's and guardians' experiences, number of meetings and identified areas for improvement.

Your skills and experience

We are looking for you as:

Has a degree in sociology or other relevant university education that we consider to be equivalent.
Has good knowledge of SoL, LVU and LSS legislation and legal practice
Has several years of current experience in social work and a good understanding of social childcare
Is used to leading meetings and creating structure
Expresses yourself clearly in speech and writing in Swedish.

You are good at collaborating with others by having an open and accommodating and problem-solving approach. As a sip coordinator, you are expected to be self-motivated and take responsibility for your work. We believe you act professionally based on your profession by distinguishing between the personal and professional. You are safe, stable and have self-awareness. You also show a holistic view by seeing the whole and taking the larger perspective into account.

Job title: Social secretary.
