* Mobile care and support in collaboration with an interdisciplinary team
* Working with standards of care
* Assessing needs, making diagnoses and planning care measures
* Guidance and instruction for trainees in the nursing and care professions
*
“Primary Nurse” function:
** Ensuring needs-based care and support by coordinating resources within the team
**Technical support for team members
** Moderation of practice team discussions
* Carrying out nursing visits and admissions
### Profile:
* Diploma in general and/or psychiatric health and nursing (nostrification if completed abroad)
* Completed registration in the health professional register
* Driving license B (own car is an advantage)
* Professional experience in (mobile) care an advantage
* Ability to communicate and enjoy working independently
