Qualified health and nursing person with the function "Primary Nurse" (f/m/d) 1 Qualified health and nursing person with the function "Primary Nurse" (f/m/d) Area of responsibility:
* Mobile care and support in collaboration with an interdisciplinary team
* Working with standards of care
* Assessing needs, making diagnoses and planning care measures
* Guidance and instruction for trainees in the nursing and care professions
* "Primary Nurse" function: Ensuring needs-based care and support through coordination of resources within the team, technical support for team members, moderation of practice team discussions, etc.
* Carrying out nursing visits and admissions

### Profile:
* Diploma in general and/or psychiatric health and nursing (nostrification if completed abroad)
* Completed registration in the health professional register
* Driving license B (own car is an advantage)
* Professional experience in (mobile) care an advantage
* Ability to communicate and enjoy working independently
